This measure from the National Institute on Drug Abuse is a brief self-report questionnaire comprised of 8 items assessing use of cannabis, cocaine, amphetamine-type stimulants, inhalants, sedatives, hallucinogens, opioids and other miscellaneous drugs, alcohol, and tobacco.
